November is Open Enrollment for Employee Insurance
Thursday, November 7th, 2013

THE ONLY TIME EMPLOYEES CAN MAKE CHANGES TO THEIR INSURANCE COVERAGE IS DURING OPEN ENROLLMENT which is during the month of NOVEMBER each year unless they have a QUALIFYING EVENT (marriage, divorce, birth of child, death, change in life status). If a QUALIFYING EVENT occurs employees are eligible to make changes on their insurance coverage at that time.

Insurance coverage for those signing up during open enrollment will begin on January 1, 2014.

Please contact Della Walsh via email or at 733-2704, Extension #9504, if you have any questions or would like to schedule an appointment to make changes or additions on any of the insurance options.

ALL PAPERWORK WILL NEED TO BE COMPLETED AND TURNED IN TO THE PERSONNEL OFFICE BY MONDAY, DECEMBER 2ND AT 5:00 P.M.. THERE WILL BE NO EXCEPTIONS.

ATTACHED YOU WILL FIND THE PARTICIPATION FORM THE FOR THE FLEXIBLE BENEFITS PLAN FOR 2014. ALL EMPLOYEES ARE TO REQUIRED TO COMPLETE THIS FORM. Also attached, for your information, is a listing of FSA Eligible Health Care Expenses and information regarding Flexshare Benefits Online. Please review this information if you are interested in having a Flexible Spending Account for Dental and Vision Expenses, or Dependent DayCare for 2014, or if you are not covered under the Teton County School District Health Insurance Plan and would like to have a Medical Spending Account. THE ATTACHED PARTICIPATION FORM NEEDS TO BE COMPLETED AND SUBMITTED TO MY OFFICE NO LATER THAN DECEMBER 2ND, 2013.

Summit Insurance will be holding meetings at the various building sites during the month of November in regards to AFLAC Insurance. Notification of meeting dates and time will be sent out by Summit Insurance. Call 733-2055, if you have questions for Summit Insurance.
